Our Life Management Services team is here to provide long-term planning and support that helps maintain—and even improve—the quality of life for your loved one and your family.
Some families have already had the opportunity to test our new Life Management Services department and share feedback on what they need most. We are excited to open this department to all HighPointe families now and begin inviting you for your first in-person meeting with our team.
Over the next several months, you’ll receive a personal invitation to meet with us. Our goal is to connect with families twice a year in person. Ideally, these meetings will happen before your annual service plan meeting with your case manager, so we can:
- Review what’s been happening at HighPointe
- Discuss what the day program looks like and whether any units need to be allocated for improvements
- Ensure you are receiving all the resources you are entitled to
- A guide in planning for the future
- Establish working goals for the year
