Planning for the long-term for a person with a disability requires planning and insight. At HighPointe Services, we know what it is like to walk the road, feeling unsure, alone, frustrated and concerned about what the future holds. We know what it is like to need help and support. While case management agencies provide guidance on accessing funding sources, they do not walk with you week by week for the years ahead. HighPointe knows that our clients and their families need more support to manage the long-term path of their adult lives.
To provide a deep and supportive relationship with our clients, the HighPointe Life Management Services team works with each client and their families and support staff to create a long-term service strategy leading the way to a more fulfilled life.

It is important to note that most of our clients attend our program over 1,500 hours per year, compared to meeting with their case management agencies 10 hours per year. The extensive interaction with our clients over the year puts us in a unique position to understand what each of our clients require for personal growth. Every client’s situation is unique. Having a good understanding of your personal growth goals is an important first step in the planning of your future. We listen to your needs, assess your situation and work with you to develop a personal growth strategy that helps achieve your goals. Whether you are just starting your journey into adult services or are already there, our guidance can help keep you on track.

We begin by getting a thorough understanding of your personal situation, goals and needs. We use this information in our proprietary multi-faceted assessment process to develop a support profile that maximizes the likelihood of reaching your personal growth goals. We believe this decision has the biggest impact on your life over time. From there, we align your ability, interests and personal growth plan with the appropriate HighPointe program track so we can help you achieve your goals. Finally, we prepare your individual support guidelines for our staff to support you as you move forward toward your goals. We believe this flexible, active approach to our client’s personal growth management enables us to capitalize on providing the best program opportunities and help you achieve your personal goals.

Keep You on Track with Our Growth Tools

As a client, you’ll hear from us regularly to make sure you're informed about your progress in our program and help ensure your personal growth plan continues to serve your personal needs. Many clients are often tempted to stray from the growth plan, which is why we focus on providing unparalleled client service. As a client, you’ll have a Client Success Advisor who provides you with regular, ongoing updates on your progress utilizing our proprietary personal growth assessment tools providing accurate measurements of the impact of our service offerings.

Provide Continuing Growth Strategies

Once you become a client, your Client Success Advisor will serve as a long-term resource for information and support. You’ll also have access to exclusive community-access resources such as our recreation programs, social clubs, personal growth video library and other communication from members of our support team.

Trusting someone else with helping you to plan your future can be overwhelming. We want your decision to become a HighPointe client to be based on an honest discussion on how we can support your personal plans. Too often, seeking support for your needs is done with a short-term perspective. People can be tempted to choose convenience over quality. Jumping from provider to provider more often leads to frustration and poor long-term outcomes.

Client Benefits

HighPointe Services was established to create a path to achieving our client’s personal growth plans. We are more than just a residential and day program service provider. HighPointe Services is committed to helping our clients and their families to plan and achieve their long-term objective of ensuring a high quality of life and a continuity of care for health management and safety.

Becoming a client of HighPointe Service will provide the following benefits:

  1. We take time to learn about your unique goals and needs.
  2. Then, we create a personal growth plan with clearly defined goals.
  3. Our Client Success Advisors then work with our Program Design team to establish a series of educational activities aimed at providing the tools and skills necessary to achieve your goals.
  4. You will have access to one of our four specialized learning tracks delivering the services identified during your personal growth planning process. 
  5. Every six months your Client Success Advisor will meet with you to review your progress utilizing our proprietary personal growth assessment tools.
  6. Once a year, we will review your personal growth plan and make any necessary revisions.
  7. Access to the HighPointe App which will include timely information about daily program activities, information about each provider working with our clients, announcements and updates, photos of activities and outings and other information designed to keep you informed.
